from Key Offices

The Cebu People’s Action Center works closely with various government offices to make sure every concern is addressed properly. From health and social welfare to education, livelihood, and emergency response. These key departments provide direct assistance, referrals, and follow-through based on the type of help needed.

  • PGO – Provincial Governor’s Office

The PGO serves as the executive arm of the Provincial Government. Through the Governor’s directive, CPAC may request:

  • Immediate financial assistance for medical, burial, or emergency needs.

  • Referral and endorsement letters for hospital admissions, legal aid, or agency coordination.

  • Medical mission deployment in underserved areas.

  • Support for community outreach programs.

  • Transportation assistance (e.g., ambulance services, official vehicle requests).

  • Special aid for calamity-affected residents or individuals in distress.

  • PSWDO – Provincial Social Welfare and Development Office

As the social welfare arm of the province, the PSWDO can provide:

  • Assistance for indigent, displaced, abused, elderly, and PWD individuals

  • Case management services for domestic violence, gender-based violence, child protection

  • Livelihood support and training referrals

  • Assistance in housing, food packs, disaster response

  • Referral to national DSWD programs (4Ps, AICS, etc.)

  • Psychosocial support services (especially for survivors or traumatized individuals)

  • Barangay Affairs Office

This office serves as the liaison between the provincial government and barangays. Through them, CPAC can coordinate:

  • Validation and endorsement of complaints or assistance requests.

  • Facilitation of barangay assembly/community dialogues.

  • Logistics for CPAC barangay caravans or mobile services.

  • Assistance in conflict resolution and barangay-level documentation.

  • Coordination of barangay data and population statistics.

DEPARTMENT SUPPORT PROGRAMS

  • Department of Social Welfare and Development (DSWD)

Support for CPAC:

  • AICS (Assistance to Individuals in Crisis Situation): medical, burial, transportation, and food assistance

  • Relief goods and emergency support during calamities

  • Social pension for indigent senior citizens, PWD assistance, livelihood support

Basic Requirements:

  • Duly accomplished DSWD application form

  • Barangay Certification/Endorsement

  • Valid ID of the client and representative (if any)

  • Medical Abstract/Certificate (for medical assistance)

  • Death Certificate (for burial assistance)

  • Police/Incident Report (for victims of calamity or abuse)

  • Department of Health (DOH)

Support for CPAC:

  • Free medicines and medical consultations

  • Medical missions, vaccination drives, health programs

  • Patient referral to hospitals under DOH

Basic Requirements:

  • Referral letter from CPAC or Barangay Health Center

  • Valid ID of patient

  • Medical abstract, laboratory requests, or doctor’s prescription (for medicines and treatment)

  • Department of Labor and Employment (DOLE)

Support for CPAC:

  • Livelihood starter kits

  • TUPAD (short-term emergency employment)

  • Skills training and job referrals

Basic Requirements:

  • Duly accomplished DOLE application form

  • Barangay Certificate of Indigency/Residency

  • Valid ID

For TUPAD:

  • Duly signed TUPAD contract and attendance sheet

  • Bank account/GCash details for payout

  • Department of Agriculture (DA)

Support for CPAC:

  • Seeds, fertilizers, and farm inputs

  • Farmer training programs

  • Livelihood assistance for fisherfolk

  • Department of Education (DepEd)

Support for CPAC:

  • Educational assistance referrals

  • Literacy and out-of-school youth programs

  • Partnership for community education activities

Basic Requirements:

  • School ID or Enrollment Form

  • Certificate of Enrollment/Assessment of Fees

  • Barangay Certificate of Indigency (for scholarships/aid)

  • Department of Trade and Industry (DTI)

Support for CPAC:

  • Livelihood and small business training

  • Microfinancing and access to capital

  • Market linkage for local products

Basic Requirements:

  • Business Registration (Barangay permit or DTI certificate)

  • Valid ID

  • Business proposal or concept note (for loans/assistance)

  • Philippine National Police (PNP)

Support for CPAC:

  • Community safety and peacekeeping

  • Handling cases of domestic violence, abuse, or crimes

  • Mediation support for disputes

Basic Requirements:

  • Police blotter or complaint affidavit

  • Valid ID

  • Supporting documents (medical certificate, barangay blotter, witness affidavit if applicable)

  • Local Government Units (LGUs)

Support for CPAC:

  • Quick-response services for health, education, and livelihood

  • Relief operations and disaster response

  • Direct financial and medical assistance through the Mayor or Governor’s office

Basic Requirements:

  • Barangay Certificate of Indigency/Residency

  • Valid ID

  • Supporting documents depending on the type of assistance (e.g., medical abstract, school assessment form, calamity report)

Other Provincial Departments and Offices

Depending on the case or concern, CPAC may also coordinate with the following:

  • PHO (Provincial Health Office)
    Medical assistance program, rural health services coordination, health referrals

  • PDAO (Persons with Disability Affairs Office)
    Disability ID processing, mobility aid support (wheelchairs, crutches), benefits application

  • PESO (Public Employment Service Office)
    Job fairs, employment matching, skills training

  • PPDO (Provincial Planning and Development Office)
    Data requests, community needs assessments, planning support

  • PGSO (Provincial General Services Office)
    Supplies or equipment support for outreach activities or official use

  • PDRRMO (Disaster Risk Reduction and Management Office)
    Response to calamity-stricken areas, disaster kits, risk assessment reports

  • Legal Division / PAO Coordination

  • Financial aid for funeral and burial expenses of indigent families

  • Educational Assistance (limited cases)

  • Some support for indigent students, depending on availability of funds

  • Emergency or Special Assistance

  • For victims of calamities, disasters, or other urgent situations

How to Avail Assistance:

  • Submit a request letter (addressed to the Vice President)

  • Prepare requirements (usually includes: medical abstract, hospital billing, prescription, valid ID, barangay certificate of indigency, and death certificate if burial assistance)

  • Processing is coursed through OVP satellite offices or partner agencies

Hospitalization / Hospital Bill Assistance

• Medical Abstract or Medical Certificate
Must be: Original, Latest copy with Doctor’s name, Signature and License number.

• Latest and Final Original Copy of Hospital Bill
Signed by: Billing Clerk

• Original copy of Justification Letter
For private room accommodation – prepared and signed by: Hospital Staff / Attending Physician / Medical Social Worker or Admitting Staff

• Original copy of Indigency from the Barangay
With name & signature of Barangay Captain with Barangay Seal.

• Original Copy of Social Case Summary Report
(Prepared by: LGU Social Worker stating the kind of assistance granted from the LGU and kind of assistance to recommend to PSWDO.)

Burial Assistance

• Original or Certified True Copy of Registered Death Certificate

• Original Copy of Funeral Contract
(Signed by both parties with contract price and remaining balance and downpayment)

• Original Copy of Statement of Account

• Original copy of Indigency from the Barangay
With name & signature of Barangay Captain with Barangay Seal.

• Original Copy of Social Case Summary Report
(Prepared by: LGU Social Worker stating the kind of assistance granted from the LGU and kind of assistance to recommend to PSWDO.)

Dialysis / Laboratory / Chemotherapy Assistance

• Original Copy of Medical Certificate or Medical Abstract

• Original Copy of Doctor’s Request
(With Doctor’s full name, address, Contact no. & Signature)

• Quotation or Charge Slip
(With supplier’s full name, address, Contact no. & Signature)

• Original copy of Indigency from the Barangay
With name & signature of Barangay Captain with Barangay Seal.

• Original Copy of Social Case Summary Report
(Prepared by: LGU Social Worker stating the kind of assistance granted from the LGU and kind of assistance to recommend to PSWDO.)

Assistive Devices (Wheelchair, Crutches, Cane, etc.)

• Medical Certificate
